What you share

The volunteer interest form asks for your name, email address, phone number, age, and permission to contact you. Please do not submit medical information or other sensitive details.

How the form works

When you submit the form, your information is sent to FormSubmit, a third-party form service, for processing and spam prevention, and forwarded to ConnectingHelpingHands@gmail.com. FormSubmit may present a CAPTCHA and retains submissions according to its own practices. Review FormSubmit’s privacy policy (PDF, opens in a new tab) before submitting. You can email us directly instead of using the form.

Why we collect it

Connecting Helping Hands uses your details to respond to your interest, communicate about volunteer opportunities, and help coordinate next steps. Submitting this form does not apply directly to a hospital or guarantee a volunteer placement.

Students and families

This form is intended for high school students ages 13 and older. Students under 18 should involve a parent or guardian when exploring opportunities and completing hospital requirements. Hospitals may require their own consent forms, screening, and training.
